- Assessment of infants without a family doc, within a week or so of discharge after birth, to ensure a successful transition to home.
- Follow-up is booked for infants who have ongoing medical problems at the first appointment.
- Most infants are healthy, and are discharged to follow-up with local health unit and medical clinic facilities until they are able to find a family doc for ongoing primary care.
